SELF-ASSESSMENTS

Our mission is to help you break free from anxiety and start building a meaningful life, one step at a time.

Taking a questionnaire offers a quick and effective way to gain immediate insights into your struggles.

Whether you’re looking to better understand yourself or deepen your connection with others, these quizzes offer actionable results in just minutes.

Take the first step in understanding yourself on a deeper level.

THE PLAYING-IT-SAFE QUESTIONNAIRE

self-assessment

  • Do you ask others what to do so you don’t make a mistake?
  • Do you naturally get stuck thinking doom and gloom scenarios?
  • Do you often feel like an imposter despite all your accomplishments?
  • Do you doubt your competency and ability to handle challenging situations?
  • Does your mind spontaneously go back to the past and dwell on troublesome situations?
  • Are you very careful about what you’re saying, how you’re saying it, and what you’re doing so you don’t make mistakes?
  • Do you do your best to think optimistically and positively even though your mind keeps coming up with negative thoughts?

If these scenarios sound familiar, you are playing it safe.

At one point or another, we all play-it-safe when feeling stressed, worried, or anxious. We overthink stuff, avoid making decisions, postpone activities or act in ways that minimize our discomfort right away.

I call those ways of thinking or acting playing-it-safe moves.

Sometimes those ways of thinking and those actions are adaptive responses. However, if your playing-it-safe moves become habitual and you don’t check how they work in your life, you are at the risk of getting stuck in your head and undermining your own best interests.

Pausing to reflect on your playing-it-safe moves and how they might be holding you back is the first step towards enjoying a more joyful and meaningful life.

In my work, I have identified 10-playing-it-safe moves that we all rely on, but when going unchecked and used often, keep us living a contradiction between the life we want to live and the life we’re living.

Take 10-minutes to complete the Playing-it-safe questionnaire and figure out:

  1. Your playing-it-safe profile
  2. Which playing-it-safe behaviors keep you stuck.

Your playing-it-safe profile will give you the awareness you need to stop living small, stop relying on ineffective playing-it-safe moves, and start living with purpose every day you’re awake.

Parenting Questionnaire

self-assessment

This comprehensive questionnaire is designed to help you identify the specific type of anxiety your child or teen may be experiencing. By taking a few minutes to complete it, you’ll gain valuable insights into your child’s emotional well-being.

Early recognition of anxiety symptoms is crucial. It allows for timely intervention, which can significantly reduce the risk of developing more complex anxiety disorders. This proactive approach can make a substantial difference in your child’s long-term mental health.

As you work through the questions, you’ll likely become more aware of your child’s emotional needs and behavioral patterns. This increased understanding can strengthen your ability to provide targeted support and create a more nurturing environment at home.

It’s important to note that while this questionnaire is a powerful screening tool, it is not a diagnostic instrument. The results should be used as a starting point for further discussion and evaluation. We strongly recommend consulting with a qualified mental health professional who can provide a comprehensive assessment and, if necessary, develop an appropriate treatment plan tailored to your child’s specific needs.

By taking this step, you’re demonstrating your commitment to your child’s emotional well-being and taking an active role in supporting their mental health journey.

Why our clients love working with us

testimonials

"Working with Patricia Zurita Ona has changed and saved my life. I have struggled off and on for years with sever anxiety disorder and OCD . I have been to therapy throughout the years and have found some helpful but nothing really got to the core of the problem. I often felt crazy and that my incredible fear and my phobias would remain with me thorough my life, I would not drive on the freeway, eat certain things and had constant catastrophic thinking, thus creating a lot of depression.

I needed Cognitive Behavioral Therapy which I saw advertised on many therapists’ websites. When I would call most of them they said they offered it and specialized with Anxiety Disorder/ OCD which was not the case. I had felt alone, hopeless and frustrated for the past 9 years. Never feeling like I was getting the help I needed. It was not until I started meeting with Patricia that I actually started to feel better and feel as if I had control over my life and started to live again ...

“Patricia and her team have completely changed my life! I cannot express my gratitude enough.

She helped me craft values and goals I wanted to have in my relationships, and walked me through each step of how to get there. Her methods are simple, manageable, and make sense when you are upset. I’ve been to therapists who have told me to take deep breaths, visualize calming colors, rip paper, go for a walk or scream in to a pillow when im angry. But in the moment, when im soooo angry, none of these world work. I would forget to do them, or I would already be so angry, that they were ineffective to help me calm down. Patricia’s methods are not only EXTREMELY effective, but they are easy to do, they make sense in the moment, and they bring about radical change when you apply them ...

“I do not know how it reflects on my credibility should I happen to brag about being a seasoned vet when it comes to therapists and mental wellness pursuits. Regardless, I am. And on top of that, I also happen to be a health care practitioner myself. This means, for the purposes of a review, that when I walk into a new Doctor’s office, there is an additional dimension of critiquing for me to process. I would never willingly put myself under the guidance and care of a Doctor who did not 1) Impress me as a capable and honest caregiver and 2) Impress me as a professional colleague.

– Empathy
– Knowledge
– Professionalism
– Respect

I think that these are the basic bulletpoints that strike me when I consider why I think so highly of Patricia. Dr. Zurita takes NOTHING more seriously than the pursuit of her profession. Except possibly you, the patient.

Some therapists really care.

Other therapists are extremely knowledgeable.

Very few therapists are both. Patricia Zurita Ona is one of those very few.”
